Job Overview

We are seeking a skilled and compassionate Care Coordinator to work collaboratively with chronically ill or high-risk patients, their family members, healthcare providers, and community resources. The successful candidate will provide timely access to care, promote preventative services, reduce emergency room visits and hospital readmissions, and enhance patient well-being.

Key Responsibilities:
  • Promote timely access to appropriate care and increase utilization of preventative services
  • Reduce emergency room utilization and hospital readmissions
  • Increase comprehension through culturally and linguistically appropriate education
  • Create and promote adherence to a care plan
  • Manage relationships to ensure continuity of care
  • Assist patients with medication refill requests and prior authorizations
  • Connect patients to relevant community resources

Requirements:

  • Bachelor's degree or equivalent
  • Three to five years of experience in clinical or community resource settings, preferably in care coordination or case management
  • Bilingual skills are highly desirable

Estimated Salary: $65,000 - $85,000 per year
