TotalMed is seeking a highly skilled Registered Nurse to join our team as a Clinical Care Coordinator in a Step Down Unit setting. This role requires strong clinical expertise, excellent communication skills, and the ability to work collaboratively with interdisciplinary teams to deliver high-quality patient care.

As a Clinical Care Coordinator, you will be responsible for assessing, monitoring, and treating patients transitioning from intensive care to general medical-surgical units.
Duties and Responsibilities- Assessing Patient Needs: Conduct thorough assessments of patients' vital signs, cardiac rhythms, and respiratory status to identify potential issues and develop effective treatment plans.
- Clinical Decision Making: Collaborate with interdisciplinary teams to make informed decisions regarding patient care, taking into account their unique needs and medical histories.
- Medication Administration: Administer medications safely and effectively, ensuring compliance with hospital policies and procedures.
- Education and Support: Educate patients and families on post-intensive care recovery techniques, medication management, and self-care strategies to promote optimal health outcomes.
- Communication: Communicate effectively with healthcare team members to facilitate patient progress, address concerns, and optimize outcomes.
