Job Overview
The Advanced Clinical Pharmacist plays a crucial role in the healthcare team, showcasing expertise in various pharmacist duties, including the interpretation, transcription, and verification of medication orders.

Engagement in daily multidisciplinary rounds is essential, where the pharmacist will contribute to drug utilization assessments aimed at promoting effective medication therapy. This position also involves leading and developing clinical pharmacy services, as well as mentoring and educating healthcare staff to ensure precise and appropriate documentation of clinical activities.

Key Responsibilities
- Lead and participate in pharmacy clinical and operational initiatives.
- Monitor patient medication regimens and implement necessary interventions.
- Review and interpret treatment orders to confirm the accuracy of dosages, treatment schedules, and routes of administration, ensuring safe practices based on laboratory data and evidence-based guidelines.

- Collaborate with various hospital departments to enhance clinical pharmacy responsibilities and support service lines.
- Develop and lead pharmacy clinical services, including but not limited to drug consultations, nutritional support, pharmacokinetics, anticoagulant monitoring, pain management, and renal dosing.

- Provide comprehensive medication education and counseling to patients, caregivers, and medical personnel.
- Work closely with medical and nursing teams to optimize pharmacotherapeutic care, including infusion and outpatient medication management.

- Supply information regarding medication uses, interactions, and potential side effects to staff and patients.
- Oversee the preparation of treatments, investigational medications, and ensure proper handling of drug returns.

- Maintain accurate records of patient prescriptions, charges, and inventory management.
- Ensure pharmacy technician duties are executed safely and effectively, minimizing any risk to patients.

- Analyze and evaluate various treatment options, making independent decisions as necessary.
- Assist in the training and mentoring of pharmacy students and residents.
- Perform additional duties as assigned.

Qualifications
Education:
A Bachelor's degree in Pharmacy or a Doctor of Pharmacy from an ACPE-accredited institution is required.

Experience:
A minimum of 3 years' experience as an Advanced Clinical Pharmacist in a specialized area or at least 5 years of general hospital pharmacy experience is required. Alternatively, completion of a Post Graduate Year 2 (PGY2) pharmacy residency or fellowship is acceptable.

Certification/Licensure:
A current state license to practice pharmacy is required, with the expectation of obtaining licensure within a specified timeframe. Basic Life Support certification is required upon hire, and Advanced Cardiac Life Support certification must be obtained within a designated period.
