About the Position
* Coordinate patient care activities, ensuring seamless transitions and efficient use of resources
* Communicate effectively with patients, families, and healthcare teams to address patient needs and concerns
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Utilize clinical knowledge and expertise to identify and address potential patient safety risks
Requirements
* RN licensure required
* BSN or higher degree preferred
* Proven track record of excellent patient care and coordination
* Strong analytical and problem-solving skills
Salary and Benefits
* Competitive salary: $140,000 - $170,000 per year, based on experience
