Clinical Care Coordinator

The Clinical Care Coordinator will play a vital role in delivering high-quality patient care to our seniors. This position requires a skilled and compassionate RN to work collaboratively with our interdisciplinary team to assess, plan, and implement care for our patients.
Key Responsibilities
* Participate in daily team meetings to discuss patient care and develop care plans
* Conduct periodic health assessments and identify acute needs and illnesses
* Collaborate with the team to develop and implement advanced care plans
* Educate patients and caregivers on health and wellness topics
Requirements
* RN degree from an accredited nursing program
* Current license to practice as a Registered Nurse in the State of Michigan
* Minimum of one year of experience working with the frail elderly or long-term care population
* Previous home care experience or long-term care experience is preferred
