Job Title: Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at Weill Cornell Medical College. As a Clinical Care Coordinator, you will play a vital role in coordinating and organizing the clinical care of patients within our practice.

Responsibilities:
  • Monitor medical supplies and ensure timely replenishment.
  • Triage patient inquiries and communicate test results in a clear and concise manner.
  • Attend unit-based in-service and continuing education programs to maintain current knowledge and skills.
  • Provide in-office patient care services, including obtaining patient histories, vital signs, and administering medications.
  • Develop and implement comprehensive plans of care based on individual patient needs.
  • Maintain inventory of supplies, equipment, and reagents, and order as necessary.
  • Evaluate patient status in emergency situations and administer appropriate care.
  • Schedule tests and procedures for patients and educate them on treatment and therapies.
  • Record diagnosis, treatment plans, progress notes, and doctors' orders in patient charts.
  • Coordinate and participate in patient follow-up care and provide continuity of care through patient family education and discharge planning.
Requirements:
  • Associate's Degree in Nursing or Bachelor's Degree in Nursing preferred.
  • Current NY State Registered Nurse License and Registration.
  • BLS Certification issued by the American Heart Association.
  • Infection Control Certificate.
Working Conditions:

Standard office work environment with occasional exposure to medical supplies and equipment.
