**About the Role**

We are seeking a compassionate and energetic MDS Coordinator to join our management team at Big Horn Rehabilitation and Care Center. As a key member of our clinical staff, you will engage with residents, learn their stories, and ensure they receive the best care in the industry.

**Responsibilities**

* Collaborate with our management team to provide exceptional care to our residents
* Utilize your knowledge of the RAI process to ensure accurate and timely assessments
* Work closely with our healthcare team to develop and implement individualized care plans
* Provide education and support to residents and their families

**Requirements**

* Registered Nurse with healthcare experience in a long-term care facility (LPN/LVN with approval from the facility NHA)
* Knowledge of RAI process required
* Knowledge of Medicare preferred

**Physical Demands and Environment**

* Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk, and hear
* Employee will occasionally assist to lift and/or transfer residents weighing 100+ pounds
* Employee may occasionally be exposed to blood or other body fluids

**Note**

Applicants extended a conditional offer of hire will be required to take and pass a drug test before commencing work. Employment offers will be withdrawn whenever an applicant receives a verified positive test result or refuses to participate in the testing process.
