Fraser is in search of a Care Coordinator who will facilitate immediate access to services and assist individuals seeking support through our organization. Qualifications:

  • Must meet at least one of the following criteria:
    • High School diploma or equivalent required
    • Bachelor's degree or higher in a behavioral science or related field from an accredited institution preferred.
    • Certification as a Community Health Worker or Family Peer Specialist preferred.
    • Strong commitment to promoting diversity, multiculturalism, and inclusion, emphasizing culturally responsive practices and self-reflection.

Key Responsibilities:

  • Facilitate mental health screenings for individuals seeking same-day access.
  • Assist clients in obtaining necessary services across various domains, including mental health, social, educational, and recreational, to enhance overall functioning.
  • Proactively identify and address barriers to accessing recommended services.
  • Maintain communication with the client's care team to ensure continuity of care.
  • Conduct same-day access appointments during designated hours.
  • Provide consultation and coordination with both internal and external providers regarding shared clients.
  • Collaborate with the Care Coordination Supervisor and Director to develop new processes for supporting families in accessing appropriate services and resources.
