We are seeking a highly skilled Community Resource Coordinator II to join our team at Fidelis Care. As a key member of our care management team, you will play a vital role in connecting our members with community resources and support services to enhance their overall well-being.
Key Responsibilities- Support members in accessing community resources and care services to address their social determinants of health needs
- Collaborate with the care team to identify member support needs and provide health education as appropriate
- Provide assistance to members with non-clinical general health assessments and referrals to appropriate care services
- Conduct outreach and engagement activities to locate hard-to-reach individuals and families in the community
- Develop and maintain relationships with community organizations and resources to ensure seamless referrals and support
- Document and maintain accurate records of member interactions and community resource information
- High School diploma or equivalent required; 1-2 years of related experience preferred
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations
- Knowledge of social determinants of health and community resources
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
