About the Role

We are seeking a highly motivated and compassionate Care Management Support Coordinator II to join our team. As a key member of our care management team, you will play a vital role in supporting our members in achieving their health and wellness goals.

Key Responsibilities
  • Provide outreach and support to members via phone to facilitate care plan next steps, community or health plan resources, and address questions or concerns related to scheduling and ongoing education.
  • Support members in connecting with other health plan and community resources to ensure high-quality customer care and service.
  • Apply working knowledge of assigned health plan activities and resources to support members.
  • Serve as the front-line support for member and/or provider inquiries, requests, or concerns, including explaining care plan procedures and protocols.
  • Support member onboarding and day-to-day administrative duties, including sending welcome letters, related correspondence, and program educational materials.
  • Document and maintain non-clinical member records to ensure standards of practice and policies are in accordance with state and regulatory requirements.
  • Knowledge of existing benefits and resources locally and make referrals to address Social Determinants of Health (SDOH) needs.
Requirements
  • Requires a High School diploma or GED.
  • Requires 1 – 2 years of related experience.
