Job SummaryThe Pediatric Emergency Care Coordinator works collaboratively with the nursing leadership team, providers, registered nurses, technicians, and pre-hospital emergency medical staff to plan, direct, and evaluate the care provided to pediatric patients and their families.
The ideal candidate will possess expert clinical knowledge, applying it systematically and continuously to collect data in collaboration with multidisciplinary team members to provide therapeutic care for pediatric patients and/or families.
Key Responsibilities:
- Communicate effectively and work collaboratively to facilitate evidence-based, standardized practice among the multidisciplinary team.
- Develop and implement care plans that meet the unique needs of pediatric patients and their families.
- Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients, families, and healthcare staff to promote health literacy and patient engagement.
- Participate in quality improvement initiatives to enhance patient care and safety.
To be successful in this role, you will need:
- At least 1 year of care coordination experience.
- A graduate degree from an NLN-accredited School of Nursing.
- A current NJ-RN License (Registered Nurse) and American Heart Association BLS Certification (Basic Life Support for Health Care Providers).
- Certain specialties may require advanced certification (e.g., ACLS, NALS, PALS).
