RN Care Manager

Ohio's Hospice is seeking a dedicated and compassionate RN Care Manager to join our team. As a member of our interdisciplinary team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities:
- Provide high-quality patient care and support to patients and their families in their homes and facilities.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on end-of-life care and related topics.
- Document patient information and care plans accurately and thoroughly.
Requirements:
- Active Registered Nurse license in the State of Ohio.
- Two years of acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred.
- CPR certification.
- Computer skills sufficient to document services and care.
- Valid Driver's License with a safe driving record.
