Position Overview: Heart Transplant Nursing Coordinator
We are seeking a skilled Registered Nurse to join our Heart Transplant Program, which performs over 100 heart transplants annually. The Heart Transplant Nursing Coordinator will play a crucial role in supporting patients throughout the transplant process, collaborating with a multidisciplinary team of healthcare professionals.
This position involves providing specialized nursing care to heart transplant patients, ensuring they receive comprehensive support from pre-transplant evaluations through post-operative care. The coordinator will be available 24/7 to address patient needs and facilitate effective communication among the healthcare team.
Key Responsibilities:
- Deliver advanced nursing care for patients across the transplant continuum, ensuring high-quality support at all times.
- Coordinate pre-transplant evaluations to optimize resource utilization and patient care.
- Monitor patient health status and determine the necessity for further assessments or interventions.
- Facilitate scheduling for diagnostic procedures while maintaining fiscal responsibility.
- Collaborate with the healthcare team to develop and implement care protocols for transplant patients.
- Educate patients and families about the transplant process, ensuring they understand all aspects of care.
- Participate in research initiatives to enhance transplant care practices and outcomes.
- Serve as a consultant for nursing staff and other healthcare professionals regarding transplant patient care.
Qualifications:
To be considered for this role, candidates must possess a Bachelor of Science in Nursing (BSN) with a preference for a Master of Science in Nursing (MSN). A minimum of five years of nursing experience, including two years in transplant or critical care settings, is required. Current RN licensure in North Carolina is mandatory.
