Key Responsibilities- Medication Management: Prepare and assist residents with self-administration of prescribed medications, maintaining accurate records and ensuring compliance with individual medication care plans.
- Resident Care: Provide supervision and coordination of resident care delivery, working closely with the Director of Nursing to set standards of nursing care and manage residents under the direction of a physician.
- Inventory Management: Maintain accurate inventory of required medications, monitor expiration dates, and properly store medications, adhering to controlled drug procedures and ensuring proper disposal of expired, contaminated, or excess drugs.
- Communication: Effectively communicate with residents, families, and healthcare professionals, ensuring seamless care coordination and addressing any concerns or issues promptly.
- Documentation: Maintain accurate and complete resident records, including medication administration records, incident reports, and resident care notes, ensuring confidentiality and adhering to regulatory requirements.
- Team Collaboration: Work collaboratively with the nursing team, residents, and other departments to ensure seamless care delivery and address any concerns or issues promptly.
- Licensure: Current LVN licensure in the state of practice.
- Education: High school diploma or equivalent required; college degree preferred.
- Experience: Minimum of three years of experience in a similar position, with retirement living or home-care background preferred.
- Skills: Excellent verbal and written communication skills, ability to work effectively in a team-based environment, strong time management and organizational skills, and proficiency in computer software, including Microsoft Office and Property Management Systems.
