Licensed Vocational Nurse/Licensed Psychiatric Technician

Telecare Corporation is seeking a skilled and compassionate Licensed Vocational Nurse/Licensed Psychiatric Technician (LVN/LPT) to join our team at CORDILLERAS MHRC, a 16-bed mental health rehabilitation center serving San Mateo County residents. As an LVN/LPT, you will play a vital role in providing high-quality care to individuals with serious mental illness.
Job Summary:The LVN/LPT will provide services related to the safe and appropriate administration of medical treatment, including medications, as prescribed by the physician. You will also be responsible for documenting client responses to treatment and notifying the treatment team of any clinical changes in condition.
Responsibilities:- Provide safe and effective implementation of direct care in accordance with established policies, procedures, and standards of care.
- Establish and maintain a therapeutic relationship with clients.
- Attend and participate in community meetings and groups.
- Monitor the activities and documentation of Mental Health Workers (MHWs) on the assigned unit.
- Coordinate and participate in counting medications and giving report at the beginning and end of each shift.
- Coordinate admission of new clients by designating the admitting MHW and completing the admission documentation.
- Coordinate discharges by ensuring that discharge orders are written and that discharge medication and clothing are given to the client.
- Transcribe physician orders accurately and in a timely manner.
- Administer medication and treatments to clients.
- Receive and present change of shift report.
- Monitor the monthly recap of physician orders, medication record, and treatment records.
- Review all medical and psychological reports and inform the physician and nursing administrator of any abnormalities found in the client's condition or laboratory reports.
- Complete the 24-hour Client Report each shift.
- One year of experience in psychiatric nursing in a skilled nursing facility or an acute hospital is preferred.
- Graduation from an accredited Licensed Vocational Nurse or Licensed Psychiatric Technician program, and current licensure by the State of California are also required.
- A valid CPR Certificate is also required.
- Necessary skills include a demonstrated understanding of psychiatric illness and its treatment, knowledge of psychotropic medications, and knowledge of the laws and regulations governing the delivery of mental health services in the State of California.
The LVN/LPT will work in a fast-paced, dynamic environment, providing care to clients with serious mental illness. The position requires the ability to work independently and as part of a team, with excellent communication and interpersonal skills. The LVN/LPT will also be required to work flexible hours, including on-call shifts, and may be required to work in a variety of settings, including the CORDILLERAS MHRC and other Telecare facilities.
