The Vocational Support Specialist will be an integral part of a collaborative clinical team at Thresholds. This role involves actively engaging clients who are interested in vocational services and facilitating job interviews and exploration to promote job retention. Responsibilities include documenting individual services and outcomes, as well as assisting clients in navigating their employment journey.
Key Responsibilities:- Ensure confidentiality of consumer records.
- Perform administrative tasks such as answering calls and managing documentation.
- Develop and oversee Individual Service Plans (ISPs).
- Identify and create suitable employment opportunities tailored to individual client needs and preferences.
- Conduct situational assessments and vocational explorations to evaluate clients' job readiness.
- Monitor and document client progress to ensure alignment with ISP goals.
- Exercise sound judgment in decision-making processes that affect service quality.
- Collaborate with referral sources and community organizations.
- Support clients in enhancing job-seeking skills, including interview preparation and resume development.
- Provide direct support to clients at their workplaces, focusing on vocational instruction and social skills development.
- Prepare and submit accurate assessment and progress reports in a timely manner.
- Maintain consumer files in compliance with HIPAA regulations.
- Encourage client involvement in their program planning and execution.
- Assist clients in accessing available resources and benefits.
- Submit attendance and placement reports to the Program Manager regularly.
- Provide transportation to clients as needed.
- Participate in professional development and training opportunities.
- Perform additional duties as assigned.
Education:
Bachelor's or Master's degree in social work or a related human services field.
Essential Skills:
Proficiency in Microsoft Word, Excel, and other relevant software.
Physical Requirements:
Ability to lift a minimum of fifty pounds (50 lbs).
