Job Summary

We are seeking a skilled Clinical Home Care Coordinator to join our team at St. Mary's Home Care and Hospice, a member of Trinity Health at Home. As a Clinical Home Care Coordinator, you will play a critical role in facilitating referrals to our post-acute homecare/hospice agency, providing education to acute care staff, and contributing to business development activities.

Key Responsibilities
  • Maintain daily contact with case managers/discharge planners and units to offer assistance and ensure timely and efficient follow-up on patient referrals.
  • Interact with healthcare team members to answer questions related to homecare/hospice and community-based services, as they plan for patient discharges from the acute care setting.
  • Coordinate care for clients going home from the hospital who have chosen our agency's services.
  • Document initial assessments of homecare needs and potential plans for services in compliance with agency policy.
  • Obtain physician orders for initiation of homecare/hospice services in accordance with agency policy/procedure and third-party regulations.
  • Interact with healthcare team members, clients, and their families to answer additional questions regarding homecare/hospice services being requested.
  • Make appropriate referrals for evaluation/care to other services as needed, and coordinate care with others to ensure effective and efficient care is provided.
  • Send client information to intake at the agency, either electronically or manually.
  • Notify hospitals of readmission and provide current information regarding client/family status.

Requirements
  • Graduate of an approved clinical education program
  • Current clinical license in the state of employment
  • Minimum of two years' experience in a professional clinical setting, preferably in acute care or home care
  • Current Driver's license and reliable transportation
