Job Summary

We are seeking a skilled LPN to join our Home Care team as a Home Care Revisit LPN. As a Home Care Revisit LPN, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional patient outcomes.

Responsibilities
  • Provide one-to-one care to patients in their homes, including assessment, care planning, intervention, and supervision of patient care and outcomes.
  • Follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.
  • Utilize advanced technology and clinical knowledge to deliver exceptional patient outcomes.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and patient satisfaction.
Requirements
  • Graduate of an accredited or state-approved school of practical nursing.
  • Licensure as an LPN in the State of Georgia.
  • A minimum one year of experience in an acute care setting; home care experience preferred.
  • Phlebotomy and wound care experience required.
  • Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
