Position Title: Clinical Home Care Coordinator

Position Overview:
  • Manage referrals to our post-acute homecare and hospice services
  • Educate acute care personnel on our service offerings
  • Engage in initiatives to foster business growth
Key Responsibilities:
  • Maintain regular communication with case management teams
  • Oversee care coordination for patients transitioning from hospital to home
  • Secure physician orders to initiate homecare services
  • Conduct initial assessments to determine homecare requirements
  • Collaborate with other service providers as necessary

Essential Qualifications:
  • Graduation from an accredited clinical education program
