Your Role as a Case Manager

Reporting to a Case Management Supervisor, you will be responsible for the full range of case management duties, including cases requiring intensive support and specialized expertise. You will implement clients' service plans and ensure they receive the necessary resources to achieve their goals.

Key Responsibilities:

  • Develop and implement individualized service plans for clients with developmental disabilities.
  • Coordinate services and support from various community agencies and providers.
  • Advocate for clients' needs and rights within the system.
  • Conduct regular assessments to monitor client progress and adjust service plans as needed.
  • Maintain accurate and up-to-date client records.

Qualifications:

  • A Master's degree in social work, psychology, or a related field is preferred.
  • Experience working with individuals with developmental disabilities is essential.
  • Strong communication, interpersonal, and problem-solving skills are required.
  • A valid driver's license and reliable transportation are necessary.
