Job Summary

Key Responsibilities
  • Conduct psychosocial assessments and develop individualized service plans to address clients' needs
  • Facilitate linkages to community-based services, including mental health, substance abuse, and housing support
  • Collaborate with multidisciplinary teams to provide crisis intervention and support
  • Develop and maintain relationships with community partners to enhance service delivery
  • Provide case management and advocacy services to clients
Requirements
  • Master's degree in Social Work (MSW) or equivalent
  • Current licensure as a Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC)
  • Strong knowledge of child and adolescent development, emotional/behavioral health, and mental health
  • Experience working with diverse populations, including homeless individuals and families
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • Bilingual or multilingual skills
  • Experience working in a crisis-driven environment
  • Knowledge of community-based services and resources
