Nurse Care Coordinator

We are seeking an Experienced Registered Nurse to join our Cardiac Care Services team at Prisma Health in Columbia, SC. As a Nurse Care Coordinator, you will provide clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient-centered care.
This is a non-management position that requires staff to be scheduled for a significant amount of shift, weekend, holiday and on-call work. The ideal candidate will have one year of experience in Nursing as a Registered Nurse and be licensed in South Carolina. In lieu of 1 year of registered nurse experience, we will accept successful completion of the Prisma Health Nurse Residency Program.
Key Responsibilities:
- Provides direct care to assigned patients in a safe, timely, appropriate and cost-effective manner; consistent with the established plan of care and identified patient priorities.
- Assesses and recognizes subtle or actual changes in patients' condition and patient response to intervention.
- Evaluates plan of care for effectiveness, documents progress toward expected outcomes, and revises plan of care as needed.
Requirements:
Associate's Degree - Nursing
One year of experience in Nursing as a Registered Nurse
Required Licensures/Registrations/Certifications
SC Licensed RN - Registered Nurse
