ROLE AND RESPONSIBILITIES
As a Case Manager, you will play a crucial role in supporting individuals with disabilities. Your responsibilities will include:

  • Conducting initial assessments to introduce families and individuals to our services.
  • Providing ongoing guidance and training to unlicensed direct care personnel.
  • Assigning qualified Direct Support Professionals (DSPs) to ensure high-quality service delivery.
  • Participating in regular meetings for all clients under your care.
  • Reviewing staff documentation for accuracy and completeness, making updates as necessary.
  • Developing and maintaining clinically appropriate objectives.
  • Collaborating with families and internal teams to facilitate smooth transitions for individuals entering our programs.
  • Building positive, trusting relationships with clients, families, and colleagues through effective communication.
  • Exhibiting strong problem-solving skills and serving as a mentor in professional ethics and standards.

QUALIFICATIONS
To be considered for this position, candidates must meet the following criteria:

  • A Bachelor's Degree in Psychology, Social Work, or a related discipline.
  • 1-3 years of experience in human services, including conducting assessments and providing in-home support.
  • Access to reliable transportation.
  • Willingness to travel within designated areas.
  • Ability to work independently and manage a flexible schedule.
