Job Summary

We are seeking a skilled and compassionate Hospice Registered Nurse to join our team. As a Hospice RN, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and advocating for the needs of our patients and their families.

Key Responsibilities

  • Provide direct patient care services to hospice patients, including assessments, care planning, and implementation.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and spiritual care coordinators, to ensure comprehensive patient care.
  • Advocate for the needs of patients and their families, ensuring that their voices are heard and their concerns are addressed.
  • Manage and coordinate patient care services, including medication management, wound care, and pain management.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in hospice care.

Requirements

  • Current RN licensure in the state of employment.
  • 1+ years of experience in hospice, home health, or palliative care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities.
  • Compassion, empathy, and a commitment to delivering high-quality patient care.
