Job Overview

Salary:

Position Summary:

The primary responsibility is to deliver personal healthcare and associated services to patients in their residences. This role involves ensuring a safe and hygienic environment, collaborating effectively with patients and their families, and communicating observations and concerns to the supervising nurse.


Supervision:

Operates under the guidance and oversight of a registered nurse.


Certification Requirements:

CPR certification is mandatory. If you are a Certified Nursing Assistant (CNA), this applies.


Educational Background:

A High School Diploma is preferred.


Qualifications:

Must have successfully completed one of the following:

  • A training program/competency evaluation as specified in Medicare regulations;
  • A competency evaluation program that meets state requirements;
  • A nurse aide training/competency evaluation program approved by the state;
  • A state licensure program that fulfills the necessary criteria.

Licensure:

Active CNA license and a current driver's license are required.


Experience:

Preferred candidates will have at least one year of experience as a Hospice Aide or Nursing Assistant in settings such as hospitals, nursing homes, home health agencies, hospices, or long-term care facilities, or have completed a Certified Nursing Assistant or Certified Home Health Aide Program.


Skills:

Must possess the ability to read, write, and verbally communicate clinical information to patients, their representatives, caregivers, and other agency personnel. A demonstrated interest in the well-being of the ill and elderly is essential. Successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test is required.


Transportation:

Reliable transportation and valid auto liability insurance are necessary.


Work Environment:

This position involves working in patients' homes under various conditions, with potential exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is required, with some exposure to adverse weather conditions.


Physical and Mental Demands:

Prolonged standing and walking are required. The ability to lift up to 50 pounds and assist in moving patients is necessary. This role may involve working under stressful conditions to meet deadlines, identify patient needs, and make quick decisions. Hand-eye coordination and manual dexterity are essential for using durable medical equipment in the home.


Key Responsibilities:
  • Deliver direct patient care under the direction of the RN and according to the Aide Plan of Care.
  • Assist patients with safe transfers and ambulation as per agency policy.
  • Report changes and document relevant information to ensure continuity of care.
  • Adhere to accepted infection control practices and maintain a clean, safe, and comfortable environment.
  • Foster positive, supportive, and respectful communication with patients, families, and colleagues.
  • Ensure respect for patient privacy and property.
  • Assist patients with proper nutrition and adequate fluid intake.
  • Contribute to the efficient operation of the agency and demonstrate effective time management skills.
  • Perform treatments and procedures in accordance with agency policy.
  • Uphold the agency's philosophy and administrative policies to guarantee quality care.
