We are seeking an experienced and compassionate Registered Nurse to join our team at Good Shepherd Hospice. As a Hospice Nurse, you will play a vital role in providing high-quality care to patients and their families in our community.
Key Responsibilities- Provide nursing assessment, planning, and care to maximize the comfort and health of patients and families in accordance with the Interdisciplinary Plan of Care.
- Initiate communication with attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Cultivate communication and bereavement skills to support patients and their families during difficult times.
- Maintain regular communication with the attending physician concerning patient/family care.
- Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the Plan of Care.
- Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
- Instruct patients and caregivers on medication administration, personal care, and other essential skills.
- Inform the Patient Care Manager of unusual or potentially problematic patient/family issues.
- Attend Hospice team meetings and other patient conferences as deemed necessary by the Patient Care Manager.
- Provide appropriate support at the time of death.
- Share in providing 24-hour, seven days a week coverage to patients and families.
- Evaluate hospice aides every two weeks and LPNs at a minimum of one time per month.
- Develop a deep understanding of hospice philosophy and pain management through individualized studies and training.
- Registered Nurse currently licensed in the state in which the care is provided.
- Hospice experience preferred.
- High integrity and ability to maintain confidential information, including all patient records.
- Strong organizational and self-management skills.
- Critical thinking and good judgment.
- Ability to work as a team player and follow defined care plans.
- Demonstrated commitment to the hospice philosophy of care.
- Valid Driver's License.
- Mileage Reimbursement.
