Key Responsibilities:
- Assess and Evaluate Patient Needs: Conduct thorough assessments of patients' physical, emotional, and social needs to develop and implement individualized care plans.
- Provide Skilled Nursing Care: Administer medications, perform wound care, and provide other skilled nursing interventions as needed to promote patients' comfort and well-being.
- Coordinate Care Services: Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Support Patients and Families: Provide emotional support and education to patients and their families, empowering them to make informed decisions about their care.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
Requirements:
- Education and Experience: Current RN licensure in the state of Texas and at least 2 years of experience as a Hospice Nurse, with a strong background in geriatric care.
- Certifications and Training: Hospice Nurse Certification (CHPN) and CPR certification, with other medical certifications highly encouraged.
- Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
