Position Overview
Under the guidance of the Program Director, the Housing Case Manager plays a crucial role in assisting adults facing serious mental health challenges and/or substance use issues within our supportive housing initiative.
Key Responsibilities:
- Facilitate the achievement of client rehabilitation objectives by applying psychosocial rehabilitation techniques.
- Establish and nurture a respectful, compassionate, and empowering rapport with clients.
- Instruct clients in essential independent living skills, including housekeeping, financial management, and resolving conflicts with housemates.
- Manage Caminar's 30-day homelessness prevention initiative, collaborating with crisis residential services to evaluate client suitability and coordinate placements.
- Maintain an assigned caseload of clients, ensuring a minimum of 65% billable service time relative to total work hours.
- Provide supportive counseling that addresses clients' immediate needs and situations, utilizing Cognitive/Behavioral interventions to help clients recognize their strengths and work towards their goals.
- Build and sustain relationships with property managers to enhance tenant-landlord interactions.
- Utilize personal or agency vehicles to transport clients to treatment locations, ensuring timely service delivery while adhering to agency vehicle policies.
- Assist clients in submitting maintenance requests and educate them on their rights and responsibilities as tenants.
- Conduct property inspections for agency client tenants to assess living conditions and communicate any clinical concerns to the housing manager.
- Oversee vendor repairs and installations as needed.
- Support clients during the moving process, including cleaning and preparing units for new tenants.
- Engage with homeless service providers to facilitate client placements in prevention programs.
- Establish connections with local board and care facilities to streamline placements.
- Assist clients in securing funds for security deposits from local charitable organizations when applicable.
- Document all objectives and interventions promptly, adhering to agency, quality assurance, and HUD standards.
- Maintain organized filing systems and contribute to an efficient office environment.
- Collaborate effectively with supervisors, case management team members, and other service providers to share critical intervention information.
- Participate in staff meetings and additional meetings as required.
- Engage in regular supervision sessions and complete assigned training in a timely manner.
- Contribute to a safe, healthy, and clean workplace in alignment with applicable laws and agency standards.
- Submit accurate timesheets and absence reports to program management punctually.
- Promote the philosophy and practice of social rehabilitation within the agency and to the public.
- Foster a cooperative and team-oriented environment that aligns with Caminar's mission and values.
- Perform other related duties and special projects as assigned.
Qualifications and Skills:
- Strong commitment to Caminar's mission.
- Preferred candidates are those registered with the California BBS; licensed supervision hours are available.
- A BA/BS in a mental health-related field is required; alternatively, a minimum of two years (4000 hours) of verified experience providing direct services to individuals with severe mental illness is necessary.
- Proficient computer skills in a Microsoft Windows environment, including email and internet navigation; experience with Human Service Information Systems is highly preferred.
- Ability to maintain confidentiality, exhibit professionalism, and represent the organization positively at all times.
- Demonstrated maturity, sound judgment, and emotional stability.
- Strong problem-solving skills, capable of identifying and resolving issues promptly.
- Excellent customer service skills, adept at managing challenging situations and responding to client needs.
- Effective oral and written communication skills, capable of conducting meetings and completing formal documentation professionally.

Physical and Environmental Requirements:
- Physical demands include occasionally pushing/pulling objects up to 50 lbs and lifting/carrying objects up to 25 lbs, with frequent typing and writing tasks.
- Must be able to walk, stand, and sit for extended periods.
- Frequent reading of documents and reports, with the ability to distinguish sounds amidst background noise.
- Cognitive demands include the ability to concentrate on tasks with frequent interruptions and remember multiple assignments over several days.
- Exposure to varied office and mixed environments, with occasional exposure to toxins and loud noises.
Special Requirements:
- Must meet criminal records clearance requirements as mandated by licensing regulations.
- A valid California driver’s license and reliable personal vehicle are essential, along with current auto insurance.
- Must pass pre-employment medical and drug tests as required by regulations.
- A personal cell phone with reliable service is necessary for business purposes.
- First Aid and CPR certification may be required.
