Job Summary:

The Clinical Partner is a vital member of our healthcare team, working under the direction of a licensed caregiver to provide high-quality patient care. This role requires a strong commitment to delivering exceptional patient experiences and a passion for staying up-to-date with the latest medical advancements.

Key Responsibilities:
  • Identify and Address Patient Needs: Recognize the physical, emotional, and social needs of patients and report findings to the RN.
  • Contribute to Patient Care Planning: Participate in the implementation of patient care and contribute to the plan under the supervision of the RN.
  • Ensure Patient Safety and Comfort: Provide for the safety and comfort of patients, families, and significant others.
Primary Duties and Responsibilities:
  • Deliver High-Quality Patient Care: Safely use restraints interventions under the direction of the charge nurse or primary RN.
  • Provide Therapeutic Communication: Offer effective communication, redirection, reality orientation, and motivation to patients to participate in activities.
Additional Responsibilities:
  • Monitor Patient Stability: Check vital signs and weight, record intake and output, and report abnormal vital signs to the charge nurse or primary RN.
  • Ensure Environmental Cleanliness: Maintain patient rooms and patient care areas in a clean and safe manner.
  • Accompany Patients for Tests and Procedures: Escort patients to and from tests and procedures as needed.
  • Assist with Activities of Daily Living: Help patients with daily activities such as bathing, dressing, and grooming.
  • Support Patient Mobilization: Assist patients with getting in and out of bed and with mobility.
  • Identify Safety Concerns: Recognize safety concerns in the patient population and notify licensed personnel.
Qualifications:
  • Educational Requirements: High School Diploma or equivalent.
  • Licenses: Certified Nurse Assistant (CNA) certification required upon hire.
  • Experience: 1 year of experience in acute, subacute, convalescent, or nursing home settings.
Physical Demands:
  • Standing and Walking: Prolonged standing and walking required.
  • Lifting and Carrying: Ability to lift and carry up to 50 pounds.
  • Pushing and Pulling: Ability to push and pull up to 75 pounds.
