About the Role:

  • We are seeking experienced Private Duty Nurses (LPN) to provide personalized care to patients in the comfort of their own homes.
  • You will work closely with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Your expertise will be invaluable in ensuring the highest quality of care and promoting optimal health outcomes for our patients.

Responsibilities:

  1. Implement prescribed care plans and administer treatments as directed by physicians.
  2. Conduct comprehensive patient assessments and coordinate care with other healthcare professionals.
  3. Document all care provided in a timely and accurate manner.
  4. Provide education and support to patients and their families on health promotion, disease management, and self-care techniques.

Requirements:

  • Degree in Nursing (LPN) and current licensure in good standing.
  • At least one year of experience in a similar role or related field.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • TB skin test within the last 12 months.
  • Reliable transportation and a valid driver's license.
