- Provide clinical expertise in patient placement and facilitate patient movement through assessment of patient needs and coordination of transfers
- Communicate effectively with staff, patients, and families to identify and resolve problems
- Serve as a clinical resource for patient care activities and problem resolution
- Assume responsibility for own professional growth and development
- Valid multi-state or State of Oklahoma Registered Nurse License
- Certification in area of specialty preferred
- 1-2 years related experience
- Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways
- Effective interpersonal, written, and oral communication skills
- Ability to organize and prioritize work in an effective and efficient manner
