Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family, a part of LHC Group. As a Private Duty LPN, you will provide direct nursing care to patients in their homes, working under the supervision of a Registered Nurse. Responsibilities
  • Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
  • Document patient visits thoroughly and completely in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
