Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family, a part of LHC Group. As a LPN Home Care Specialist, you will provide direct nursing care to patients in their homes, working under the supervision of a Registered Nurse. This is a great opportunity for a dedicated and compassionate nurse to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Provide high-quality clinical services to patients in their homes, following the plan of care and in coordination with the healthcare team.
  • Document patient visits thoroughly and accurately in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information and maintain a professional demeanor at all times.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
