Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Manitowoc. As a Private Duty LPN, you will provide high-quality nursing care to patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, as prescribed by physicians
  • Record clinical and progress notes, and coordinate other patient care activities to provide the highest quality of patient care
  • Work under the direct supervision of a Registered Nurse and/or designated supervisor
  • Ensure confidentiality of patient health information
Requirements:
  • Current LPN licensure in the state of practice
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
  • Current CPR certification
  • Previous home care experience preferred
