Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family, a part of LHC Group. As a LPN Home Care Specialist, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, as prescribed by physicians.
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Ensure compliance with state laws governing the practice of nursing, agency policies, and procedures.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification.
Preferred Qualifications:
  • Previous home care experience.
