Job Summary

We are seeking a dedicated and compassionate Social Services Specialist to join our team. As a key member of our care team, you will assist residents and families in adjusting to placement, illness, and plan of care, ensuring the highest practicable level of functioning.

Responsibilities
  • Conduct interviews and gather information from residents and families to develop psychosocial histories, assessments, and care plans.
  • Assist residents and families with transition and adjustment to placement.
  • Determine legal status of residents upon admission and ensure that resident and family wishes regarding treatment are implemented.
  • Inform residents and families of their personal and property rights.
  • Provide input on social, psychological, and emotional needs of residents and families to supervisors.
  • Ensure residents have required evaluations for visual, audio, mental health, and dental needs.
  • Arrange residents' appointments, including transportation.
  • Provide assistance to residents and families in finding and utilizing financial, legal, mental health, and other community resources.
  • Participate in resident and family council meetings as requested.
  • Develop a working knowledge of federal and state regulations related to the position and provide support with applications and eligibility information requests.
  • Support resident theft and loss prevention programs.
  • Work closely with Ombudsperson to resolve resident rights questions or concerns.
  • Perform documentation duties in accordance with Company charting and documentation policies and procedures.
  • Assist in discharge planning services, including arrangement for follow-up services, transfers to other facilities, and post-discharge plan of care.
Qualifications
  • Previous experience with geriatric populations or in long-term care settings is strongly preferred.
  • High school diploma or equivalent is required; AA or Bachelor's degree is preferred.

  • Ability to represent the company in an ethical and professional manner and maintain accountability.
  • Ability to interact tactfully, effectively, and professionally with other employees, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be able to relate to and work with physically and/or mentally ill, disabled, elderly, emotionally upset, and at times hostile people within the facility and treat them with respect and consideration.
