Key Responsibilities:
- Develop and maintain expertise in clinical pharmacy practice, including the assessment and management of patients with complex medication regimens.
- Collaborate with healthcare teams to optimize medication therapy and prevent medication-related problems.
- Provide accurate and timely medication information to healthcare providers and patients.
- Participate in quality improvement initiatives to enhance patient safety and outcomes.
- Develop and maintain strong relationships with healthcare providers and patients to ensure seamless care transitions.
- Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
- Contribute to the development and implementation of pharmacy policies and procedures.
- Provide education and training to pharmacy staff and students.
Requirements:
- Pharmacist license issued by the state in which you practice.
- Bachelor's Degree in Pharmacy.
- Experience in hospital pharmacy preferred.
Preferred Qualifications:
- Strong computer skills and experience with electronic health records.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong problem-solving and critical thinking skills.
Working Conditions:
- Must be able to stand for long periods of time.
- Will occasionally lift up to 35 lbs. without assistance.
- Must have functional speech, vision, and hearing.
- Will be exposed to chemical hazards.
