Home Health Physical Therapist Opportunity

We are seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care to patients in Pueblo, Colorado.

About the Role

As a Home Health Physical Therapist, you will work with patients who have lost mobility, strength, and functionality due to illness or injury. You will provide personalized therapy in the comfort of their own homes, helping them regain their strength, manage pain, and reach their goals.

Responsibilities
  • Provide home-based physical therapy to patients with a focus on personalized care and goal-oriented treatment
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, COTAs, PTAs, and SLPs, to develop and implement patient care plans
  • Conduct assessments, observe movements, and document progress to modify care plans as needed
  • Educate patients and their families on therapy goals, home exercises, and self-care
Requirements
  • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
  • Minimum of three years of physical therapy experience, ideally one year in home healthcare
  • CPR Certification (demonstration course required)
  • Knowledge of state and federal home health regulations
