Primary Care Provider

The Primary Care Provider plays a vital role in delivering high-quality patient care in the comfort of their own homes. As a key member of our healthcare team, you will conduct thorough health assessments, manage acute and chronic conditions, and provide personalized care plans tailored to each patient's needs.
Responsibilities:- Conduct comprehensive health histories, physical examinations, and lab tests to identify potential health issues.
- Develop and implement treatment plans, prescribe medications, and refer patients to specialists as necessary.
- Ensure accurate and timely documentation of patient encounters, including SOAP notes, within our Electronic Health Records System.
- Provide education and counseling to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Collaborate with other healthcare professionals to develop and refine our service line's Standards of Care.
- Maintain accurate records of patient data, adhering to federal and state guidelines.
- Communicate effectively with patients, families, and colleagues to ensure seamless care transitions.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current State license as a Registered Nurse and Advanced Practice Nurse.
- Experience: Two years' experience as an APN, clinical care setting preferred; hospice or home health experience beneficial.
- Nursing skills aligned with generally accepted standards of practice.
- Excellent interpersonal and communication skills.
$120,000 - $150,000 per year, based on experience, reflecting the organization's commitment to attracting and retaining top talent.
