Primary Care Physician Lead

At CenterWell Senior Primary Care, we are dedicated to delivering exceptional patient care and improving the quality of life for our patients. As a Primary Care Physician Lead, you will play a vital role in shaping our medical services and setting the tone for our practice.
We are seeking a skilled and compassionate physician to join our team as a Primary Care Physician Lead. In this role, you will oversee the clinical operations of our center, ensuring that our patients receive high-quality care and service. You will work closely with our Regional Medical Director and Center Administrator to develop and implement policies and procedures that promote excellent patient outcomes.
Your responsibilities will include:
- Evaluating and treating patients in accordance with standards of care
- FOLLOWING LEVEL OF MEDICAL CARE AND QUALITY FOR PATIENTS AND MONITORING CARE USING AVAILABLE DATA AND CHART REVIEWS
- ASSISTING IN THE COORDINATION OF PATIENT SERVICES, INCLUDING BUT NOT LIMITED TO SPECIALTY REFERRALS, HOSPITAL AND SNF COORDINATION, DURABLE MEDICAL EQUIPMENT AND HOME HEALTH CARE
- ACTION AS AN ACTIVE PARTICIPANT AND KEY SOURCE OF MEDICAL EXPERTISE WITH THE CARE TEAM THROUGH DAILY HUDDLES
- HELPING REGIONAL MEDICAL DIRECTOR AND CENTER ADMINISTRATOR IN SETTING A TONE OF COOPERATION IN PRACTICE BY DISPLAYING A PROFESSIONAL AND APPROACHABLE DEMEANOR
- COMPLETING ALL MEDICAL RECORD DOCUMENTATION IN A TIMELY MANNER WORKING WITH A QUALITY-BASED CODER TO OPTIMIZE CODING SPECIFICITY
- FOLLOWING POLICY AND PROTOCOL DEFINED BY CLINICAL LEADERSHIP
- MEETING WITH RMD ABOUT QUALITY OF CARE, REVIEW OF OUTCOME DATA, POLICY, PROCEDURE AND RECORDS ISSUES
- PARTICIPATING IN POTENTIAL GROWTH OPPORTUNITIES FOR NEW OR EXISTING SERVICES WITHIN THE CENTER
- PARTICIPATING IN THE LOCAL PRIMARY CARE ON-CALL PROGRAM OF CENTERWELL AS NEEDED
- ASSURING PERSONAL COMPLIANCE WITH LICENSING, CERTIFICATION, AND ACCREDITING BODIES
- Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
The estimated salary range for this position is $250K - $350K depending on experience and location.
