Job Summary:

The East Boston Neighborhood Health Center seeks an experienced RN Care Manager to join its Project SHINE clinical care management team. This role presents a unique opportunity to make a profound impact on the lives of individuals living with HIV/AIDS and those at risk for HIV transmission.

About the Role:

This position requires flexibility, adaptability, and a strong ability to work with high-need communities. The RN Care Manager will provide comprehensive nursing care, develop individualized patient care plans, and promote adherence to treatment through outreach strategies. They will also demonstrate leadership skills in a multi-disciplinary care team setting.

Responsibilities:

  • Demonstrate proficiency in HIV-specific nursing care, completing thorough assessments, and providing follow-up care as needed.
  • Develop and implement patient care plans, including adherence and outreach strategies, in collaboration with the PCP/ID Specialist and other team members.
  • Care for patients living with HIV, teaching them about chronic disease management, adherence, STI screening, and safe sex practices.
  • Assume a leadership role in the multi-disciplinary care team, promoting teamwork and self-direction.
  • Interact with patients and staff in a professional, respectful manner, maintaining a positive attitude and using communication devices effectively.
  • Work cooperatively with the Counseling and Testing team, performing intake STI assessments and risk assessments, managing STD symptom screening, and referring patients for adequate care.
  • Coordinate treatment for patients with positive STD results, ensuring follow-up care and referrals as needed.
  • Provide specialized nursing support for medical providers, interpreting lab results, and diagnosing new HIV infections.
  • Evaluate and follow up on patients receiving NPEP (post-exposure prophylaxis) and PrEP (pre-exposure prophylaxis) treatments.

Requirements:

  • High School Diploma required.
  • Completion of vocational/technical training beyond high school in an Accredited Nursing Program required.
  • Licensure in Massachusetts as an R.N. or L.P.N. required.
  • BCLS certification (per departmental policy) required.
  • Two years' experience in HIV nursing preferred, preferably in a community health setting.
  • Previous experience working with substance abuse and/or HIV Care patients.

Salary: $39.85 - $61.76 per hour, depending on experience, plus a $10,000 sign-on bonus.
