We are seeking a skilled RN Care Manager to join our team at the East Boston Neighborhood Health Center. This is an exciting opportunity to make a profound impact on the lives of people living with HIV/AIDS and those at risk for HIV transmission.
About the Role
The RN Care Manager will work as an integral member of our clinical care management team, utilizing their expertise in HIV-specific nursing care to provide comprehensive support to patients. This role requires flexibility, excellent interpersonal skills, and the ability to work with high-needs communities. Outreach methods may vary based on organizational needs and may include telephonic or in-person interactions in various settings.
Key Responsibilities
- Provide thorough clinical follow-up, addressing care gaps, vaccinations, and annual screenings.
- Develop individualized patient care plans, including adherence and outreach strategies, in collaboration with the PCP/ID Specialist and other HIV team members.
- Promote teamwork through self-direction, self-motivation, and problem-solving skills, while working cooperatively with others at all levels of the organization.
- Care for and educate patients on topics related to chronic disease management, adherence, STI screening, and safe sex.
- Implement HIV nurse-on-call procedures, including liaison work with inpatient facilities, the on-call physician, and mental health administrator.
- Safely administer medications, tests, and treatments as ordered, managing medication refills with appropriate authorization.
- Document patient histories, assessments, plans, and treatments thoroughly and accurately.
Requirements
This position requires:
- A High School Diploma and completion of vocational/technical training beyond high school in an Accredited Nursing Program.
- Licensure in Massachusetts as an R.N. or L.P.N.
- BCLS certification (per departmental policy).
- Two years of experience in HIV nursing, preferably in a community health setting, and previous experience working with substance abuse and/or HIV care patients.
Benefits
We offer a competitive salary range of $39.85-$61.76 per hour, depending on experience, along with a $10,000 sign-on bonus (pro-rated for part-time and per diem positions).
