Job Summary:

We are committed to providing safe, quality, and best-in-class care for individuals living with Opioid Use Disorder (OUD). Our team members are engaged in medication-assisted treatment, counseling, support, and care management to address the underlying causes of OUD and provide a continuum of care that supports patients on their journey to recovery.

Key Responsibilities:
  • Complete patient intakes, admissions, discharges, and transfer paperwork
  • Document patient progress through counseling and interaction
  • Develop and implement individualized treatment plans
  • Identify clinical/case management needs and address them
  • Perform individual, group, and family counseling
  • Report patient abuse, neglect, and exploitation as required
  • Report patient grievances as required
  • Educate patients on treatment, health issues, and steps to recovery
  • Obtain Urine Drug Screens and patient photo identification
  • Assist in monitoring patient activities on center premises
  • Participate in community relations activities
  • Ensure compliance with 42 CFR Part 2 and 45 CFR, Parts 160 & 164
  • Maintain proper licensure and/or certification
  • Participate in preparation for surveys and inspections
  • Attend staff meetings
  • Ensure compliance with local, State, Federal, and Colonial Management Group, LP rules, regulations, and policies
Requirements:
  • Education/Licensure/Certification: Qualified candidates will have a counseling certification in a related field (State Specific)
  • Experience: Minimum of 500 hours of experience in substance abuse is preferred
  • Skill and Ability: Excellent interpersonal and communication skills, ability to multitask, prioritize, and be dependable and reliable
