Job Title: Home Health Physical Therapist

NeoGenCare is seeking a skilled Physical Therapist to join our team in Kapolei, Hawaii.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for planning and administering prescribed physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities.

Responsibilities:
  • Develop and implement individualized physical therapy treatment plans to achieve maximum results.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Provide one-on-one physical therapy sessions to patients in their homes.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care.
  • Document patient progress and maintain accurate records.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • Current and unrestricted license to practice Physical Therapy in the State of Hawaii.
  • Current CPR/BLS certification.
  • Minimum 1 year of recent Physical Therapy experience in an institutional setting.
  • Minimum 1 year of recent Physical Therapy experience with a home health agency.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
