Job Summary

Benchmark Human Services is seeking a skilled Licensed Practical Nurse to provide high-quality nursing care to critically ill infants, children, and young adults in the comfort of their own homes.

About the Role

As a Licensed Practical Nurse with Benchmark Human Services, you will have the opportunity to work independently, free from the limitations of a large hospital or long-term care facility. You will provide skilled nursing care to clients, following established plans of care and nursing care plans.

Responsibilities

  • Provide professional nursing care as defined in the nurse practice act and under the direction of a RN.
  • Communicate appropriate status/condition changes to the RN Clinical Care Manager or designated RN.
  • Provide skilled nursing care for clients as directed by the RN Clinical Care Manager.
  • Follow the Plan of Care and Nursing Care Plans established by the physician and RN.
  • Recognize and report changes in client condition to the RN Clinical Care Manager and physician, as directed.

Requirements

  • Valid Indiana Nursing License.
  • Valid driver's license.
  • Willingness to travel to nearby clients in need of assistance.
