Job Summary

The MDS Coordinator is responsible for coordinating care plans in accordance with regulatory requirements. This role ensures that important resources are made available to patients and that patient care is delivered effectively and to a satisfactory standard.

Key Responsibilities

Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, guidelines, policies, and procedures.
Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental, and psychosocial needs as identified in the resident's assessment.
Review complaints made by residents or their families regarding the resident assessment or care plan and document the grievance, actions taken, and current status of the complaint.
Participate in surveys (inspections) made by TJC and other authorized government agencies.
Develop and maintain good relations with other departments, providers, facilities, and agencies that may be involved in the resident assessment/care plan functions.

Requirements

Education level equivalent to completion of high school, and completion of a minimum Practical Nurse Training course of study. Registered Nurse level is preferred.
Must have and maintain an unencumbered Georgia Licensed Practical Nurse license, or Registered Nurse License and Basic Life Support CPR certification.
One to two years related experience and/or training preferred.

Work Environment

The MDS Coordinator will work in a fast-paced and dynamic environment, requiring self-motivation, creativity, and capabilities to function in a semi-autonomous role.
