Job Summary

The Nurse Care Manager position at CoxHealth is responsible for coordinating patient care to ensure a quality, cost-effective plan. This involves conducting comprehensive assessments, identifying gaps in care, and developing treatment plans to meet health care goals. The successful candidate will work collaboratively with the Population Health team to measure, improve, and maintain quality outcomes for individual patients and the population served.

Key Responsibilities

  • Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs.
  • Identify gaps in care and barriers to attaining improved health.
  • Develop treatment plans to meet health care goals for individuals.
  • Collaborate with physicians and staff to ensure effective care coordination.
  • Monitor and follow-up on patients' progress regularly.

Requirements

  • Graduate of an approved nursing program.
  • Preferred: Bachelor's Degree in Nursing, Health Sciences, or other related field.
  • Preferred: 2-3 years of nursing experience in home health, ambulatory care, community public health, or case management.
  • Excellent telephone and interpersonal communication skills.
  • Ability to multi-task and prioritize; strong organizational skills.
  • Strong knowledge of Medical Terminology.
  • Computer experience required.

Licensure/Certification/Registration

  • MO RN license required.
  • BLS must be obtained within 90 days of hire.
