Job Summary

Under the general direction of the MDS Manager, the MDS Coordinator is responsible for coordinating care plans as per regulatory requirements. The incumbent must ensure that essential resources are made available to patients and that patient care is delivered effectively and to a satisfactory standard.

Key Responsibilities
  • Coordinate the development and completion of the Care Center resident assessment in accordance with current rules, regulations, guidelines, policies, and procedures.
  • Monitor and ensure care plans are adhered to, properly documented, and residents receive the best care in accordance with TJC, federal, state, and local guidelines, organizational and departmental policies and procedures.
  • Communicate with medical staff, other departments, and outside agencies while maintaining confidentiality.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, filing, etc., as necessary and sign and date them.
  • Review complaints made by the residents or their families regarding the resident assessment of care plan and document the grievance, actions taken and current status of complaint.
  • Watch for any sign of abuse and report all concerns to appropriate managers, CEO, or agencies.
  • Ensure appropriate team members/health care professionals are involved.
  • Develops, implement and maintain an ongoing quality assurance program for the resident assessment/care plans.
  • Assists the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.
  • Participates in surveys (inspections) made by TJC and other authorized government agencies.
  • Acts as Chairperson of the Interdisciplinary Care Plan Team and develop a comprehensive resident assessment and care plan for each resident and make written or oral reports as necessary or requested.
  • Participates/serves on various committees to include Quality Assurance, Utilization Review and any others you may be appointed to and implement recommendations to/from the various committees and MDS, resident care plans & assessments.
  • Participates in discharge planning as necessary.
  • Develops and maintains good relations with other departments, providers, facilities and agencies that may be involved in the resident assessment/care plan functions, keeping abreast of changes in policy and seeking advisements to further the quality of care.
  • Conducts/Coordinates the interview for each resident's assessment, evaluating condition and medical data, special activities, or amendments to the admission assessment, with participation from their families or outside members of the care plan team when necessary.
  • Ensure that the care provided is in accordance with the resident's wishes.
  • Provides direct nursing care as needed.
  • Participates in continuing education, and provide continuing education as regards MDS, assessments and care plans.
Requirements
  • Minimum Level of Education: Education level equivalent to completion of high school, and completion of a minimum Practical Nurse Training course of study. Registered Nurse level is preferred.
  • Formal Training: Management skills with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and verbal and written communication.
  • Certification, Registration: Must have and maintain an unencumbered Georgia Licensed Practical Nurse license, or Registered Nurse License and Basic Life Support CPR.
  • Experience: One to two years related experience and/or training preferred.
