Job Summary

We are seeking a skilled Registered Nurse to join our team at LHC Group. As a Registered Nurse, you will provide high-quality patient care and work collaboratively with our healthcare team to deliver exceptional patient outcomes.

Key Responsibilities

  • Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
  • Coordinate total plan of care with other healthcare professionals involved in patient care, ensuring continuity and quality of care.
  • Develop and implement individualized plans of care, in collaboration with physicians and other healthcare professionals.
  • Conduct regular evaluations and re-evaluations of patient nursing needs, as warranted by changes in condition or every 60 days.
  • Ensure patients meet home health eligibility and medical necessity guidelines, as defined by payer sources.

Qualifications

  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
