Job Summary

As a Clinical Care Coordinator at Phaxis, you will play a critical role in identifying and prioritizing client care needs based on client history and nursing assessment. Your expertise in client triage, assessments, and symptom monitoring will ensure timely and effective intervention and treatment.

Key Responsibilities

Perform client triage and assessments to identify acute withdrawal and intoxication symptoms
Monitor clients during medication administration and ensure adherence to unit policy and protocol
Complete required documentation in a timely and accurate manner
Deliver shift-to-shift report and communication to support consistent client follow-up
Participate in quality improvement projects and take direction from immediate supervisors

Requirements

Strong nursing background and experience working with homeless populations
Excellent communication and interpersonal skills
Ability to work in a fast-paced, community-based care setting
